Appointment & Cancellation Policy
At Cascade Medical Spa & Tattoo Removal Center, appointment times are reserved exclusively for each patient. To ensure fairness to all patients and protect provider schedules, the following policies apply.
48-Hour Cancellation Requirement
We require 48 hours’ notice to cancel or reschedule any appointment.
Appointments canceled with less than 48 hours’ notice, or missed appointments (no-shows), will result in a cancellation fee charged to the card on file:
• $50 for appointments under 45 minutes
• $100 for appointments 45 minutes or longer
This policy applies to all appointment types unless otherwise specified in writing.
Courtesy Exception
Each client is provided one courtesy late-cancellation exception.
This courtesy exception does not apply to:
• Same-day cancellations
• Missed appointments (no-shows)
Additional late cancellations may result in enforcement of fees at the discretion of the practice.
Card on File Requirement
A valid credit or debit card is required to reserve appointments.
Your card is not charged at the time of booking. It is only charged if a cancellation or no-show falls within the policy window outlined above.
Required Intake Forms
All required intake forms must be completed within 24 hours of booking.
This includes:
• Cancellation & No-Show Policy acknowledgment
• Card-on-File Agreement
• Treatment-specific consent forms
Failure to complete required forms within this timeframe may result in cancellation of the appointment.
Emergencies
We understand that unforeseen emergencies may occur. Patients experiencing legitimate emergencies are encouraged to contact the clinic as soon as possible. Situations may be reviewed on a case-by-case basis at the discretion of the practice.
Refund Policy
Our providers thoroughly discuss treatment options, expected outcomes, risks, and benefits prior to treatment.
Completed Services
All completed treatment services are final. Refunds are not provided once services have been rendered.
Prepaid Packages & Unused Service Values
Unused service values from prepaid packages may be applied toward other services for which the client is clinically eligible, as determined by the provider.
Unused service values:
• Are not refundable in cash
• Are not transferable to another individual
Skincare Products
All skincare product sales are final.
If a client experiences an adverse reaction, the product may be returned within 7 days of purchase for evaluation and possible refund at the discretion of the practice.
Injectable Treatments
All injectable treatments are final once administered. Refunds or credits are not provided after treatment has been completed.
Patient Communication Authorization
By providing your contact information, you authorize Cascade Medical Spa & Tattoo Removal Center to contact you regarding your care.
This may include:
• Appointment reminders
• Consultation follow-up
• Billing communications
• Treatment follow-up
• Customer service communications
• Marketing communications (if you have provided consent)
Communications may occur via phone, voicemail, email, U.S. mail, or text message.
You may opt out of marketing communications at any time.
